Sports Physicals Always Only $30 !! ($10 donated to your sports program during the months of June and July and specified scheduled days, call to schedule)

It is also used by many other organizations including many private schools and many youth leagues. It is the responsibility of the athlete and parent to bring the correct form to have the physical performed. This is the only form available at the urgent care.)
